Abstract

Although fire and explosion incidents on ships account for only a small proportion of the total number of reported maritime accidents, they often result in severe consequences for human life, property, and the marine environment. Previous studies have confirmed that human factors play a central role in the majority of maritime accidents; however, systematic analyses of human-related causes in shipboard fire and explosion incidents remain limited. This study applies the Human Factors Analysis and Classification System (HFACS) in combination with global statistical data and accident investigation reports from the period 2005-2024 to identify, categorize, and assess the influence of human factors within the causal chain. The findings reveal that operational errors, preconditions such as fatigue and inadequate training, and insufficient organizational supervision are critical links contributing to such incidents. Based on these insights, the study proposes several measures aimed to prevention and mitigation of shipboard fire and explosion risks in the future.
